My Thoughts, Feelings & Reactions

A free one-page reflection worksheet for older primary school pupils to explore what happened in a situation, how they felt, what they were thinking and how they responded.

With six simple questions and space to draw or write, the worksheet can support conversations about thoughts, emotions and actions. It is intended for optional, adult-supported reflection when a child is ready to talk. It is not a clinical assessment or therapeutic intervention.

How to use this worksheet

Choose a quiet moment when the pupil is calm and willing to reflect.

Explain that thoughts, feelings and actions are different, but they can influence one another.

Begin with what happened, allowing the child to describe the situation in their own words.

Use the remaining questions as conversation prompts rather than expecting every box to be completed.

Some children may prefer to talk through their answers, draw pictures or have an adult write down what they say.

The final question provides an opportunity to explore possible next steps. It should not be used to suggest that a child is responsible for another person’s hurtful behaviour.

Children should not be pressured to disclose personal experiences or change their account to produce a more positive answer.

If a child describes bullying, harm or a safeguarding concern, follow the school’s established procedures rather than treating the worksheet as the response.

Who might find it useful?

This resource is particularly suited to older primary school pupils, approximately 5th–6th Class, with appropriate adult support.

It may be useful during individual or small-group work on emotional understanding, including inclusive classroom support.

Although included in SEN & Inclusion, it is not exclusively for pupils with additional educational needs. It can be adapted to suit different communication, literacy and learning needs.
