It is free to download, there is no sign-up, and you can print it as many times as you need. Suitable for all classes.

How to use it in class
Use it later the same day or the next morning, never in the heat of it. Filling it in beside the child rather than across a desk from them changes the whole tone, and it is the last box that matters, because it gives you both something concrete to point back to.
